Is it for ease of entry for the less athletic and or elderly? My Pinzgauer's are quite high off the ground and they have a step underneath but serious off roaders remove them to prevent them getting smashed. What they do to replace the step, is make one that hooks onto the lower door frame and pull it up into the cab with a piece of rope once in the vehicle. Rope operated pull up steps used to be the norm on combine harvesters. Mind you they have cabs, air con, fridges these days so probably have moved on since then.
