IMBSB - I'm also suffering from a total lack of memory recall today!!

 

In my day every vehicle had a records wallet (see attached pic's) inside which was a form that was completed as a record of every trip made (date/destination/mileage etc) inc POL issued/purchased. Together with POL carnets, toll warrants and the FMT-3 accident forms

 

Can any one recall the name/number of that form - and better still - would any one happen to have a sample copy of one???

Thanks Clive!! Don't suppose you've got a copy of the Sect 1 through 4 part have you, that can be scanned???

I have got some REME inspection forms somewhere (forgotten the official designation). I made myself unpopular by filling them in indicating the vehicle was condemned & sticking them on vehicles at a show once. I've got a wad of them somewhere, but don't worry I can't reach up to your windscreen.
